2015 HOA/LMH Payment
Election Options

If you would like to make changes to your payment election for
2015, now is the time to make that change. If you are happy
with your current election, there is nothing you need to do.
The change form is available online at
:
• Log in with Member ID and Password
• Click on Site Index
• Click on Forms
• Scroll down to CA Forms and Applications
• Under Homeowners Association Dues, click on Payment
Election and Bank Draft Authorization forms for 2015.

by email, if requested, at
The three options for 2015 HOA/LMH payments are:
1. Annual payment in full by credit card, cash or check―due
Friday, January 30, 2015.
2. ACH bank draft quarterly
3. ACH bank draft monthly
December 15, 2014 is the deadline for 2015 changes.
All statements will be mailed the first week of January 2015
with no statement fee. Remember, all mailed statements after
January 2015 will incur a $2 processing fee; to avoid this fee
you can sign up for E-Bills (see below).

E-Bills – HELP US GO GREEN!!!
The CA offers E-Bills that include your balances in all ac-
counts, including fitness classes, fitness lessons, prepaid MGA,
gift cards, sweeps, golf lessons, punch cards, and annual/combo
pass activity. Once you sign up, you will no longer receive a
mailed statement. So, help us go green, save some paper and
